AWAKE: A Long-term Follow-up Program for Survivors of Childhood Cancer

We provide comprehensive care, treatment summaries and late effects education to survivors of childhood cancer up to 39 years of age. A psychosocial team is available to assist with school issues, vocational rehab, and care coordination. Our goal is to promote healthy living and prevention or intervention to alleviate late effects.
